Insulin is a complex, large-protein molecule produced by spe­cialized “beta” cells in the islets of Langerhans in the pan­creas. The following information briefly explains the importance of insulin. This is only a basic guide. Specifically, insulin inhibits a process called glycogenolysis (the breakdown of animal starch) and gluconeogenesis (the formation of new glucose from protein). In this way the blood glucose is lowered through an alternate pathway.

Understanding insulin and its actions within the human body is crucial to understanding diabetes.

There are three basic categories of insulin: short-acting, inter­mediate-acting, and long-acting. Each is distinguished by its onset of action, peak, and duration. Onset means the time it takes the insulin to begin exerting its effect. Peak is the time insulin is working its hardest to lower blood glucose. And duration refers to how long it lasts.

The most recent development in this area is a new form of short-acting insulin: lispro. This insulin analog (a close mole­cular cousin to insulin) is sold under the brand name of Humalog. It starts working within minutes, permitting diabetics to eat right after injection.

Action times vary greatly from person to person, and between animal insulin and human insulin.

Lispro has its onset within 5 minutes and peaks in one hour. Its usual dura­tion is two to four hours. Regular has its onset within 30 to 60 minutes and peaks in two to four hours. Its duration is four to six hours. This insulin is clear and requires no rolling action to ensure proper mixing within the vial. Regular insulin tra­ditionally has been used in conjunction with other insulin types. Today, it is popular to use regular insulin alone in four to six injections spread over a 24-hour period.

Generally, proper injection time of Regular insulin is 30 to 60 minutes before meals. With lispro, you can start eating almost immediately. (The manufacturer recommends eating within 15 minutes after injection.) This allows an onset inter­val before the insulin acts. Onset will then coincide with the ingestion of your meal, making the action of the insulin as close to normal as possible.

Intermediate-acting insulin, known as NPH or Lente, begins to work in 60 to 90 minutes. Peak action is at 8 to 12 hours, with duration of up to 24 hours. This insulin type is cloudy, and the vial must be carefully rolled in the palms of the hands to ensure good mixing. Never shake this type of insulin to mix it. Insulin is a delicate protein and should never be vigorously agitated. Shaking also produces frothing or bub­bles that make it more difficult to withdraw.

Long-acting insulin, known as Ultralente, has its onset at 6 to 10 hours, peaks in 12 to 18 hours, and has a duration of 20 to 30 or more hours. Ultralente supplies a basal level of insulin and must be used with multiple doses of regular insulin. This insulin also is cloudy, and the vial should always be carefully rolled in the palms of the hands to ensure proper mixing.

A single vial preparation of premixed insulin, made up of regular insulin and NPH intermediate-acting insulin, is typi­cally available in a 70:30 ratio—70 parts of NPH intermediate to 30 parts of regular short-acting insulin. If you were to take 30 units of this insulin, using the 70:30 ratio, you would be taking 9 units of regular insulin with 21 units of NPH. Pre­mixed insulin is also now available in a 50:50 ratio.

I recommend carefully weighing the convenience of the' single vial approach against the versatility of mixing regular and NPH insulin in separate vials. The premixed preparation is difficult for the beginner and sometimes even for the vet­eran diabetic to work with. By all means, check with your doctor and be sure you understand this insulin well before you and your doctor agree to use it. The premixed insulin is cloudy because it contains NPH. The same care is needed as with other cloudy-appearing insulins.

Remember, regular insulin is always clear. If it appears slightly cloudy, don't use it. How does a blind person know which insulin is which? Take a close look at the shapes of the various vials of insulin. They are shaped differently to help you make sure you're taking the right amount and the right type of insulin.

Fitness is crucial for all soccer players. The most important fitness components in soccer include aerobic capacity, flexibility, strength, power and speed. From these fitness components aerobic capacity and strength can be improved the most by physical training, whereas genetics play a large role in determining flexibility, power and speed. The focus of this discussion will be on fitness tests and training to improve a player's fitness for soccer (with an emphasize on physical fitness and not on soccer skills).

Fitness testing for soccer
Testing of a soccer player's conditioning should be done regularly so that the effects and progress of training can be measured. Ideally a fitness testing battery would be designed for a team (or for individual players if their roles in the game are significantly different) and performed before and after training blocks of two to three months.

In soccer a player's aerobic capacity is essential for all roles other than goalkeeper. The most appropriate test to measure aerobic capacity are time trials or the beep test. The time trial would be between 3 and 5 kilometres of continuous running at maximal effort. The beep test which is effectively a shuttle run of 20 metres back and forth to exhaustion is more specific than a time trial for soccer fitness as it also demands a good degree of agility.

Strength is important in contests for the ball and running/kicking form. Tests for strength would include maximal strength tests where the player lifts as much weight as they can in a single effort. These would be focused on the core muscles abdominals, hamstrings and quadriceps.

Fitness scores in aerobic capacity and strength should be the focus of training and improvement but the following fitness components may also be tested:
Flexibility static stretches of hamstrings
Power maximum vertical and horizontal jumps
Speed sprints eg. 10, 50 and 100 metres
Agility timed around a course of cones

Training for soccer
After testing players fitness with the above tests the results should be analysed to determine their strengths and weaknesses. The focus of their training should then be to maintain (or improve) their strengths and turn their weaknesses into strengths. Most of the training should involve running (say 60-70%) at various speeds (developing aerobic capacity, power and speed), gym work for strength (10-15%), and flexibility (5-10%). The goalkeeper would probably focus more on agility, power and speed with much less emphasise on aerobic capacity. Drills to improve soccer skills and tactics would be integrated into the program so that fitness and skill level are developed at the same time.

Here is a very brief example of a training program for a soccer player:
Off-season (1-2 months):
- easy running (1-2 days per week)
- strength work (1-2 days per week)
- speed/power development (1-2 a week) (can be done following easy run)
Pre-season (1-3 months)
- easy run (2-4 a week)
- strength work (3-4 days per week)
- speed/power (2-4 a week)
- flexibility, agility, skills should be incorporated into sessions
In season (5-7 months)
- soccer game (1-2 a week)
- easy runs (3-5 a week)
- speed power (3-4 a week)
- flexibility and agility worked on every second day
- skills (6-7 days a week)

Soccer fitness is important and as has been discussed it can be improved significantly by training. To ensure training has a positive impact on a players condition always test their fitness before and after blocks of training and come game day they should be at their best.

There is a huge shortage of nurses and health care staff. By the year 2020 when over half of the the nation's nurses will be retiring or will have already retired, leaving hospitals and health care facilities with a major shortage of skilled staff. That is part of the reason there has never been a better time to become a health care worker. With a big heart, a caring attitude, and a little time spent for training you can be on your way to a rewarding career in health care.

1. Certified Nursing Assistant

Certified Nursing Assistants or CNA's used to work only in nursing homes taking care of the elderly. Now, that's not the case! There has never been a better time to become a nursing assistant. With the shortage of health care staff, CNA's are used in: hospitals, caring for seniors and disabled in their homes, Assisted Living and Residential Care facilities, and sets the foundation for advancement into other areas of health care.

CNA's perform a variety of tasks including, but not limited to: assisting patients with eating, bathing, help with dressing, assisting to the bathroom, changing incontinent patients (Incontinent: cannot hold their urine and/or bowels), documentation of patient care.

Training to become a CNA varies from state to state. Some states require that you attend a class, which is usually divided between class work and on the job training that is designed to perform newly learned skills. It normally takes only a few weeks to a few months to become a CNA.

2. Certified Medication Assistant

Certified Medication Assistants, or CMA's, administer medications to patients under the supervision of a nurse, often a Registered Nurse. In states that utilize CMA's, one usually has to work as a CNA for several months up to a year to be eligible to take the addtional class to get the required certification. CMA's are generally only used in nursing homes and assisted living facilites. The primary function of a CMA is to lighten the load of the nurse on staff and CMA's, but CMA's are not nurses and should not perform duties that are reserved only for nurses.

3. Caregiver

Caregivers do the same thing as CNA's but don't have to go to school or take a class to become one. Caregivers primarily work in home settings taking care of disabled people and elderly, and in assisted living facilities. If you have a caring attitude and genuinely want to help people, just apply at one of your local facilities. A caregiver does the same thing as a CNA: Bathing patients, feeding them, dressing assistance, incontinent care, etc… Some Assisted Living facilities will help their caregiving staff pay for the training needed to become a CNA.

4. Phlebotomist

These are the health care workers who poke you with a needle to take your blood (taking your blood is commonly called “drawing” your blood.) It doesn't usually take long to become a phelbotomist, anywhere from a few weeks to a few months. Hospitals, clinics and doctors offices are the places that keep phlebotomists on staff. When you are not drawing blood you will generally fill up your time with other lab duties.

5. Medical Assistant

Medical Assistants work in clinics and doctor's offices performing a variety of tasks. MA's, as they are normally called, will often check patients in, take them back to exam rooms, take their vital signs (blood pressure, temperature and pulse), sometimes give injections among other things. Education to become an MA depends on your state. Some states require you attend a formal school that will last anywhere from 6-9 months to two years; and some states do not require you to have a license at all. Sometimes a doctor will hire a person and train them to be an MA.

This is not intended to be a full list of all the jobs in the health care field; this is just an overview of a few of the fastest growing occupations. If you don't know where to go for information or training for any of these careers, start by checking with your local employment office. Nursing homes are good places to go too, and some of them even offer CNA classes. Hospitals and your local college will also have information about these rewarding careers.

I permanently think that Children have to do sports daily. Sport promote increase and leads to improved your health. Today, investigations shows that the significance of physical activity in Children is stronger than ever. For example, medical investigator have observed that highly energy are less likely to suffer from cancer later in life. Exercise is also known to relieve stress. Some kids experience as much depression, and anxiety as adults do. And because sport improves health, a fit child is more likely to be well-rested and mentally sharp.

Even moderate physical activity has been said to improve a child's skill at reading. The important idea of adult and coaches is to help children look through the success in sport people need to make them feel valued and needed. Every child can be successful at one sport or another. Take the time to find the sports that are right for each child. Sport is an important learning area for children. One way children get acceptance by their peers is to be the best at sport valued by other children.
